Knee pain often stems from a predictable compensatory sequence driven by gravity and structural constraints. When the body loses its ability to achieve relative motion, the tibia and femur fail to align properly, leading to excessive twisting and uneven load distribution. This misalignment frequently manifests as medial knee pain, though prolonged external rotation can shift focal pressure to the lateral aspect. Patellar discomfort often results from these underlying rotational strategies and excessive muscle activity, which compress the kneecap against the femur, rather than being a primary tracking issue. Over time, chronic load can even induce structural changes in the bones themselves, necessitating targeted interventions like orthotics or specific movement patterns to strip away these compensatory habits and restore natural, pain-free function.
